A Special Update on TRANSFORM HF’s Strategic Planning Process

2025 has been an action-packed year for TRANSFORM HF. We celebrated new teams collaborating on early-stage research at the intersection of digital innovation and equity. We launched our new patient engagement in research education program, fostering a ‘research ready’ community of people with lived experience. We hosted our largest TRANSFORM HF Network Event – a milestone that celebrated our 5th anniversary and all the groundbreaking work that has come out of the network since 2020.
Notably, we also sunsetted our 2020-2025 Strategic Plan – the compass that has guided our work since inception. As we set our sights ahead to the next five years of TRANSFORM HF, we acknowledge there’s still lots of work to be done to address the grand challenge of inequity in heart failure care. But we’re committed to what we know is working: bringing together multidisciplinary teams, supporting capacity building at multiple levels, and fostering innovation from early-stage ideas through to real-world implementation.
Mapping TRANSFORM HF’s Future
When we began our strategic planning efforts for TRANSFORM HF 2.0, we knew the diverse perspectives and experiences of our network were integral. Over the last nine months, we tapped into our most valuable resource – you! – to ensure our next steps are reflective of the interests, needs, and priorities of our community.
If you completed our member survey this past spring, shared your perspectives at our May Network Event, or took the time to provide us with feedback at any point along the way – you helped shape the vision for TRANSFORM HF 2.0. Your insights continue to be foundational to our cause, helping us craft our revised mission, vision, and goals for our future. Here’s a snapshot of our findings:
As we embark down this road, we are guided by the values that have been with us since the start – plus some new ones too! Incorporating your input, we’ve created a new set of core principles. They express the values and commitments that shape the way we collaborate, approach innovation, and measure success.
We’re also excited to work towards two new horizon principles. They represent where we want to go, challenging us to think boldly, experiment, and push beyond what’s currently possible.
1. Holistic Health: We seek to build solutions that see the whole person, not just the condition.
2. Global Reach: We want to see our breakthroughs benefit people everywhere, not just in our own backyard.
Charting the Path Forward
With the help of you – our diverse membership – we’ve also redefined our research priorities for TRANSFORM HF 2.0, concentrating our efforts on six key focus areas to meaningfully address inequity in heart failure care.
Together, they span the development of cutting-edge digital health technologies, the integration of these tools into connected systems of care and implementation models, and a commitment to equity and sustainability that ensures digital health solutions serve all communities while protecting future generations.
