Connected Care: From Wearables to Chatbot-Supported Heart Failure Management
Hear from TRANSFORM researchers who are working to scale personalized cardiac care solutions!
Dr. Saiful Hoque will share early findings from his collaborative research which aims to develop smart garments that sense and change temperature, with the ultimate goal of improving cardiovascular health outcomes in older adults with chronic conditions. Next, Dr. Pedro Velmovitsky and Dr. Dina Kanaan will present their Medly Chat project, which focuses on the production of a nurse chatbot – co-designed with patients, caregivers, and health care providers – to address patient queries and provide heart failure self-management support after hours. Lastly, Will Gao will share his research on utilizing biometric data from the Apple Watch to predict more complex measures of cardiopulmonary health in heart failure patients.

Heart Health for All: Leveraging AI and Co-Design to Bridge Gaps in Care
Prepare to be inspired by TRANSFORM members who are reimagining heart failure care through innovation at the intersection of AI, access, and equity.
Dr. Mali Worme will speak to her research team’s early findings assessing the feasibility of deploying an AI-guided point-of-care ultrasound education program in James and Hudson Bay region to support early detection of heart failure and improve outcomes in these communities. Next, we’ll hear from Kim Crasta and Dawn Donaldson as they share outcomes from their research project aimed at improving clinic workflow efficiency in outpatient settings through data analysis and an AI predictive tool. Finally, Tselot Tessema will share how her collaborative team is using data science and community-based participatory approaches to map patient journeys to heart failure treatment and engage communities in the co-design of equitable solutions.
