About Us

TRANSFORM HF was launched in 2020 to take on one of today’s biggest health challenges – heart failure. Created in joint support and partnership with the University of Toronto and the Ted Rogers Centre for Heart Research, we are bring together experts, innovators, and communities united by one goal: to make heart failure care more equitable. 

We’re developing a range of digital health tools that help diagnose, monitor, and manage heart failure more effectively. But technology alone isn’t the answer. Digital solutions only work when they’re designed with and for the people who use them. That’s why we focus on co-design – partnering with patients, caregivers, and clinicians to build tools that meet real-world needs.

Commitment to Equitable Care

Equity in healthcare requires equity within our team of healthcare professionals, academics, researchers, patients, communities, and clinicians. We believe the success of our network is dependent upon integrating inclusion, diversity, equity, and accessibility into all that we do.  

We cannot know the issues and needs of people experiencing inequities without establishing true, transparent partnerships with them. We aim to build a fully inclusive TRANSFORM HF community at all levels – making decisions, designing research, and developing treatments together.
